FLA OHT connects thousands to health care providers

Offers interim options for those still waiting

The Frontenac, Lennox & Addington Ontario Health Team is helping residents find primary care providers while ensuring they have access to health services in the meantime.

More than 6,000 people have been connected to People-Centred Health Homes, a new model offering team-based care close to where residents live. Health Homes serve specific neighbourhoods, providing convenient access to family doctors, nurse practitioners, and specialized care.

For those without a primary care provider, services are available at Health Homes, walk-in clinics, pharmacies, and community programs. These options offer prescription refills, wellness checks, mental health support, and more.

Residents waiting for a primary care connection are encouraged to register with Health Care Connect by calling 1-800-445-1822 or visiting the program’s website.

The Team's Executive Director Dr. Kim Morrison says the team is committed to connecting every resident with a provider and ensuring they know how to access care when needed.

The FLA OHT continues to work with partners to increase capacity across the region.
