Kingston Community Health Centres is helping increase access to team-based primary care
Kingston Community Health Centres (KCHC), in collaboration with the Frontenac, Lennox, and Addington Ontario Health Team, is enhancing access to team-based primary care for over 8,000 residents.
With funding from Ontario Health and support from community partners, KCHC is establishing a new Health Home in Kingston. This facility will provide comprehensive primary care and serve as a community hub for health services, including cancer screenings, prenatal and well-baby care, and sexual health services.
The Health Home currently employs a team of Nurse Practitioners, Physicians, Medical Secretaries, and Registered Practical Nurses. Plans include expanding the team to 30 members, adding professionals such as a Certified Diabetes Educator, Registered Nurse, and Social Work/Mental Health Counselors.
Starting in early 2025, students and residents from Queen’s University will also participate, enhancing care through interprofessional educational experiences.
The Health Home has a temporary location at 791 Princess Street, with plans for a permanent site. Services available in summer 2024 will focus on those without a primary care provider, including medical care for urgent health issues, prenatal and well-baby care, and sexual health services.
