“We want to ensure that community members who are waiting to be attached know where to access care”
The Frontenac, Lennox & Addington Ontario Health Team is working to ensure every resident in the region is connected to a primary care provider.
The FLA OHT has compiled a comprehensive list of health resources to help those without a provider access care.
Residents can use services such as Health Homes, walk-in clinics, pharmacies, and community outreach programs for general health services, mental health support, and specialized care. Over the past year, more than 6,000 individuals have been successfully attached to People-Centred Health Homes, which provide accessible, team-based care close to home.
“We want to ensure that community members who are waiting to be attached know where to access care,” said Dr. Kim Morrison, Executive Director of FLA OHT.
Residents seeking primary care providers are encouraged to register with Health Care Connect, the provincial service that helps people find a doctor or nurse practitioner, by calling 1-800-445-1822 or visiting the website.
